If you are experiencing chest pains or are in need of immediate assistance during non-business hours,  please call 911 or go to the nearest urgent care/emergency center.

Appointments

Once you are signed up for our office portal, you may request medication refills, request appointments, and send messages to your physician using our office portal.

For our St. Clair Shores location: Please call during normal business hours (8am - 5pm Monday - Friday) to schedule regular appointments, renew your prescription, or ask routine questions regarding your treatment. Please have your insurance information available when you call to schedule an appointment. Our telephone lines are open weekdays from 8:00 am to noon and 1:00 pm to 5:00 pm.

For all our other office locations: Please call these offices during normal business hours (8am - 4:30pm Monday - Friday) to schedule regular appointments, renew your prescription, or ask routine questions regarding your treatment. Please have your insurance information available when you call to schedule an appointment. Our telephone lines are open weekdays from 8:00 am to noon and 1 pm to 4:30 pm.

In case of an emergency after business hours, please call our answering service at 586-498-0440 and they will call the physician on duty. One of our physicians is on duty 24 hours a day, including weekends and holidays.

Your First Visit To Our Office

On your first visit to our office, we ask you to bring your medical records, insurance cards, and any medication you are taking. Prior to your visit we will mail you a new patient registration and medical history form.  You may also print off a copy of the form to complete from the link below.  We ask that you bring these completed forms with you to your first office visit.

Payments

We accept many types of health insurance. Our helpful staff will tell you if we participate in your particular health plan. We also accept VISA, MasterCard, and Discover.

We ask that you pay your copay or any balance on your account at the time of check-in.

Cancellations

If you cannot keep an appointment, please call us at least 24 hours in advance so we can give your time to another patient.

Refilling your Prescriptions

Please note:  We do not accept calls or faxes from pharmacies for refill requests.  For quicker refills, please log in to your portal access to request your refills.

Patients may either call the office for refill requests or request refills directly through our office portal access.  When calling the office to refill your prescription, please have the phone number of your pharmacy available. Please allow 72 hours for your prescription to be filled.
